This ties into an existing catch basiu & <br />we will add a new catch basin Type t(if OK w/manufacturers recommendation & will <br />correctly accommodate 24" dia pipc) or Type 2 if r.ot properly matched. Wc will place pipc <br />on properly compacted, compressible material & the onlv insnection nceded will be thc cncis <br />of thc nine to obscrve the eroutcd joints inside & outsidc thc catch basins and that the ninc <br />has not bcen cnished 0.5% (minl nitch. ¢ood workmanship, of course. Otherwise you do <br />not need to see the trcnch open. <br />Under pemiit # PW OGO6-023---We will cut & relocale thc drivcway off Madison Ave. & <br />suUscqucntly fill back to existing gradc the curb & guttcr in front of 1720 Madison (now <br />part of 1730 Madisun.) We arc to cut the curU & gutter to the appro dimensions as spccified <br />on perntit # PW 0607-021 (Uwg # G2). We are to cut curb & gutter completely in the 2 <br />areas (ncw driveway location & old location) and pour curb & gutter back to existine� <br />concrete elevations, kcying in to cxisting. Then we shnll cut ACP back 12" from gutter to <br />suffice a clean joint �atcit back in to existin� ACP elevations (wl�icli is +2" over guttcr.) <br />If you could kindly initial �C, date the 2 above points & fax back to 425348.9050 [ would apprcciatc <br />it.
