The Iceal building off icial <br />or [he State Deparunent of Labor and Industries is responsibla for electricai �onswcuon permitting. <br />Enclosed is your set oCstampcd "Appraved" plans. These plans should ba•ome a pertnanent part of the faciliry records. <br />Also enclosed are two notification cards. Please complete and retum the �reen card when consWction begins.
