According ?o our records there has been no inspection, although an attempt was made <br />on 5/18/98 (copy of repoR enclosed.) Without inspection or request for inspection the reierenced <br />permit is scheduled to expire I 1/26/98. If you wish to prevent the permit fram expiring without <br />documentation of completion, you may: <br />• Request an inspection of work that has occurred <br />• Request final inspection if the work is completed <br />• Submit a written request for a 180 day extension <br />If the permit is allowed to expire, the file will be sent to our Records Department for <br />microfilming without final approval.
