The permit file notes a phone call from you on April 16, 1996, indicating that <br />you would be �ending a wri[ten request to extend the permit. Although I have not received <br />you request, I am writing now to inform you that the permit has been extended to October 16, <br />1996. It may be maintained in an active status by requesting inspecGon at least once every 1S0 <br />d�ys.
