With the addition of the following, the plans are <br /> approved. <br /> 1. The Health District operating permit application process must be compieted prior to opening for <br /> business. <br /> 2. An indirect waste is requirod for the food preparation sink, mechanical dishwasher, ice machine, <br /> running water dipper well, anc:equipment in which food is placed. <br /> 3. A food preparation sink is required. The sink must be listed by the Nadonal Sanitation Foundation <br /> (NSF), and have at least one integral drainboard. <br /> 4. Water heaters mrst be of sufficient size to provide hot water to dishwasher and/or scullery sinks and <br /> at the same time provide hot water to all handwash sinks. <br /> 5. All food service equipment must be listed by the National Sanitation Foundation (NSF) for its <br /> intended use. <br /> 6. Storage refrigerarors must be listed for the storage of potentially hazazdous foods. <br /> 7. The Cold Tec refrigerator, item #5 on the equipment list, model #J25RR21B, is not in the cu�rent <br /> NSF listings or equivalent listings. This unit must be replaced with NSF or equivalent listed <br /> equipment or documentation must be submitted which demonstrates NSF equivalency. <br /> Manufacturers names and model numbers for replacement units must be submitted prior to the <br /> preoperational inspection. <br /> 8. Extra wall protection is required on walls behind all sinks and food prepazation tables. A 16 inch <br /> high backsplash of plastic laminate, fiberglass reinforced plastic or equal is acceptable. Floor to <br /> ceiling protection is required on wall behind dishwashers. <br /> 9. Plumbing must meet state and local codes. <br /> 10.The ventilation system shall be installed and operated to meet applicable building, mechanical, and <br /> fire codes. <br /> A preoperational inspection is required prior to opening for business. At the time of inspection the <br /> construction of the food service establishment must be complete and all equipment must be in place. <br /> Incomplete construction may result in a $130.00 rcinspection fee. Please contact the Food Program <br /> oftice a minimum of one week in advance to schedule an appolntment. This will ensure compliance <br /> with the Rules and Rep�lationc of the State Board of Health for Food Service Sanitation. <br />
