I made a point to check with the <br />owner, John Sugars, and he confirmed the fact that you �.id <br />install a necti� service and a 20 K[a electric furnace. There <br />was no pentrox on the aluminum termination, I also noticed <br />there was no permit taken for this job nor was there any <br />request for an inspection. A1r. Sugars stated you also <br />installed a furnace across the street 5706 for John H�r3yks. <br />We did find whera you took out a permit y12071 for this <br />job, but did not call for an inspection. <br />Would you please show cause by June 17, 1974 why you shoul3 <br />not be penalized a double permit fee for not obtaining in <br />electrical permit before the work was started and also for <br />not calling fo� an inspection on these two places. <br />RespectfullY <br />L.
