The Act also requires that an EIS be prepared <br />for all najor actions significan[ly affecting the qualitq of the environment. The purpose <br />of chis c?�ecklist is to help the agencies invol•�ed determine vhether oz not a proposal is such <br />a maJoi action. <br />Please ansver the following questions as completely as you can crith the infor�ation presently <br />available to you. Flhere explanations of your answers are required, or crhere you believe an � <br />esplanation would be helpful to government decision makers, include your explanation in the <br />soac� provided, or use additional pages if necessary.
