NGINEERING / PUBLIC SERVICES DEPARTMENT <br />January 23,2007 <br />Ken North <br />1812 Holbrook Ave <br />Everett, WA 98203 <br />RE: STATUS OF YOUR CITY OF EVERETT PERMIT <br />Job Address: 1812 Holbrook Ave <br />Permit: M0412-012 (Gas conversion) <br />(including All, forced air system, range, fireplace and water heater) <br />(You must reference both permit number and address when calling or writing) <br />According to our records there has been no request for inspection in the last 180 days. Please see <br />the enclosed inspection activity report. The International Building Code and the International <br />Residential Code state that all permits expire if the work authorized by the permits is suspended <br />or abandoned for a period of 180 days after the work is commenced. Work under this permit <br />will no longer be authorized unless you: <br />1) Demonstrate continuing progress by requesting inspection of work that has occurred since <br />your last inspection, or <br />2) Request final inspections of the work that is completed, or <br />3) Submit a written request for a 180 day extension, detailing justifiable cause for extension. <br />Please respond %, 'thin 10 days. After that time your permit file will be sent to our Records <br />Department for microfilming without documentation of final approval. These records are <br />available to prospective buyers and lending institutions. If you wish to resume work in the <br />future, a new permit must be obtained.
