A copy of the sketch showing <br />the approved work is enclosed. All work must conform to this sketch. <br />We are also enclos,ng a copy of our rules and regulations governing <br />work on City Streets. <br />Please notify us when you start this work, as we meat inspect the work <br />before concrete is poured. <br />This permit is valid for a period of 60 days.
