FIELD REPORT <br />NELSON Project: Fox Residence File No.: 8592B13 <br />GEOTECIINICAL Client: Logan Fox Date: 4/10/2013 <br />ASSOCIATES, INC Location: 4626 Harbor Lane Report No: 03 <br />Everett, WA <br />17311 135"' Ave. NE NA-500 Weather: Overcast/Showers, 50's page: 1 of 2 <br />Woodinville, WA 98072 Purpose of <br />(425)486-1669FAX481-2510 Visit: Construction Monitoring By:LSB/JPH <br />We visited the site today at the request of Logan Fox to observe and evaluate construction activities <br />associated with the Ultra Block wall at the residence located at 4626 Harbor Lane, Everett, WA. <br />Upon arrival, we met with Brian with Ponderosa. We observed that they had began to place the Ultra <br />Blocks within the northern portion of the wall as shown on the attached site plan. We observed that the <br />wall foundation drain consisting of a 4-inch perforated PVC pipe wrapped in drain rock and filter fabric <br />had been placed behind the base of the wall blocks. The roof drain tightline pipe consisting of a 4-inch <br />solid PVC pipe was also placed behind the wall on top of the wall footing drain. These drains then <br />daylighted through the wall in between the noiihern and soulhcm sections of the wall. These pipes were <br />sleeved with heavy duty sewer pipe whcr^ they will be under the wall and backfilled with 5/8-inch <br />minus crushed rock. The base block for the southern wall will be placed on the compacted crushed rock <br />above the pipes. The pipes will be directed to the drywell to the southwest of the wall at it 2 percent <br />grade. An approximately 12-in layer of 2-inch washed drain rock was placed behind the completed <br />blocks. Brian informed us that they would continue placing the base blocks for the southern portion of <br />the wall. <br />We have been providing monitoring services on a part-time basis only. Our findings and conclusions <br />are based on construction activities observed while on site. Based on our part-time observations in the <br />field, communications with the contractor, our understanding of this site and project plans, and provided <br />that the contractor used consistent materials and methods while we were present and not present at the <br />site, it is our opinion that the Ultra Block wall is being constructed in accordance with the plans and our <br />recommendations. <br />n <br />Attachment: Site Plan Signed: <br />Distribution: <br />
