DCUENGINEERS <br />D'AMATO CONVERSANO INC. field report <br />DATE: 11.16-07 JOB M 07.21-054 <br />PROJECT: Bethel Church WEATHER Rainy <br />LOCATION: 7505 Ha deson Road, Everett, Washington <br />GENERAL CONTRACTOR _Paul Botdaterhko <br />PRESENT: Paul Botdanenko Craht Stainer <br />On November 15, 2007, Craig Stainer of DCI Engineers visited the et Church project in Everett, <br />Washington. The purpose of the visit was to perform as a special inspector for the installation of the <br />anchors into the existing concrete walls. <br />The installadon of the anchors for attaching the new straps at the top of the wall on the north and south <br />ends were observed. These anchors are detailed in our "Out of Plane Wall Attachment Structural <br />al <br />Caclulations". The anchor installed was a 5/8" diameter threaded rod with 4" embedment and Powers <br />ACI00 adhesive. These anchors were installed per the manufacturer's instructions. <br />Also observed were the placement, size, depth and the cleaning of the holes for the new %" diameter <br />expansion anchors for the stage framing.
