BACKGROUND <br /> The mud/debris flow occurred on January 1,1997,and was observed by Jim Johnson, <br /> Senior Geologist with Golder,during a site visit on May 7,1997,for the purpose of <br /> � Preparing the proposal for this work and discussing the project approach with you. <br /> �, Mr.]ohnson had pieviously made a site visit,on behalf of your insurance mmpany <br /> � Pemm Insurance on January 3,1997. During that site visit we rernmmended that the <br /> , faIlure be mvered with plastic sheeting to protect it from fiuther erosion. We also <br /> mcommended that a geotechnical mnsultantbe contacted to investigate the failute in <br /> "� more detail and provide recommendations far rnmediation of the slope failure.
