Your permit E0203-006 is expiring and permit CO203-001 will expire on March 24, <br />2003. Copies of the last inspections are enclosed. If you wish to prevent the permiu from <br />expiring without document:uion of completion, you may: <br />1) Request inspections of work that has occurred, to document continuing progress, <br />or 2) Request final inspectio�s of the work that is completed, <br />or 3) Submit a written request for a 180 day extension. <br />Please provide the permit number when requesting inspections or exte�sions. Please respond <br />beforc Apri124, 2003. After that time the file will be sent to our Records Department for <br />microfilming without final approval.
