With the addition of the following, the plans :ire approved. <br /> 1. The Health District operating permit application process must be completed prior to opening for <br /> business. <br /> 3. Copy of inenu must be submit[ed to Health Uisuict prior ro opening for business. <br /> 3. An additional handwash sink is required next to the prep table/sink(Item#5 on Sheet K-1). <br /> 4. Relocate handsink by reach-in refrieerator (Items #I and #2 respecdvely on Sheet K-1) to the wall <br /> across from the prep table/sink (Item#l5, Sheet K-O. <br /> 5. A reduced pressure bncktlow prevention device is required at the end of the copper water pipe serving <br /> thc pop dispensing system prior to the carbonation device. <br /> 6. A horizontal separation of at Ieast l6 inches or a vertical p�v�tition 16 inches in height is required <br /> behveen the food preparation sink and all other sinks. <br /> 7. Water heaters must be of sufficient size to provide hot water to dishwasher and/or scullery sinks and at <br /> the same time provide hot water to all handwash sinks. <br /> 8. All food service equipment must be listed by the National Sanitation Foundation (NSF) for its intended <br /> use. The refrigerated package display case (Refcon model #SWINDS072) is not in the current NSF <br /> listings. This unit must be replaced with an NSF listed unit. <br /> 9. Stora�e refrigerators must be listed for the storase of potentialiy hazudous foods. <br /> l0. C-xaa wall protection is mquired on walls behind sinks and food preparation tables. A l6 inch high <br /> backspiash of plastic laminate, tiberglass reinforced plastic or equal is acceptable. Floor to wall <br /> protection is required behind dishwashers. Plastic coared hardboard is not acceptable. <br /> ( L The ceiling above the tuod prepara[ion areas must be nonabsorbent, smooth, and easily cleanable. <br /> 12. Ail li�ht fixwres in food preparation and stora�e amas must be provided with covers or shatter proof <br /> bulbs. <br /> 13. Plumbing must meet stare and local codes. <br /> 14. The ventilation system shall be installed and uperated to meet applicable building, mechanical, and fite <br /> codes. <br /> r� prcoperational inspection is required prior to opening for business.
