Checklist Review �12ll <br />Planning Eile U80-29 <br />PROPOSAL: <br />To construct a kidney clinic at the northwest corner of 33rd St. and Nassau Avenue. <br />RECOMMENDATIONS: <br />It is the recammendation of the Pubiic Works Department that issuance <br />of a Negative Declaration be subject to the following: <br />1. A detailed drawing shall be submiited to the Public Vf�orks Department <br />showing parking layout, landscaping, driveways, utilities, storm dreinage, <br />and ali required improvements to the public right-of-way. Public Works <br />pepartment approval of these drawings is required before any permits <br />are issued. All improvements shall be completed before the Occupancy <br />Permit is issued. <br />As-builts, easement, and a one year Warrantee Bond in the amount of <br />10% of the cost of construction of the public water syst: m are required <br />withln 10 v:orking �ays of obtaining public water connection. <br />2.
