Checklist Review I/211 GTYOF EVEPFE'TrT'ECC �T � qSNiNGTON <br />Planning File //80-29 <br />PROPOSAL: <br />To construct a kidney clinic at the norihwest corner of 33rd St. and Nassau Avenue. <br />RECOMMENDATIONS: <br />It is the recommendation of the Public Works Department that issuance <br />of a Negative Declaration be subject to the following: . <br />i. A detailed drawing shall be submitted to the Public Works Department <br />showing parking layout, landscaping, driveways, utilities, storm d:ainage, <br />and all required.improvements to the public right-of-way. Public Works <br />Department approval of these drawings is required before any permits <br />are issued. Ail improvements shall be completed before the Occupancy <br />Permit is issued. <br />As-5uilts, easement, and a one year Warrantee Bond in the amount of <br />10�5 of the cost of construction of the public water system are required <br />with:n 10 working days of obtaining public water connection. <br />2. City streets are to be kept clear of dirt and debris during operation. <br />3. During construction, all truck traffic to and from the site within the City <br />of Everett shall be on arterial streets unless otherwise approved by the <br />Public Works Department. <br />4. Standard street improvements in conformance with city ordinances are <br />required along the frontage of the site, street vacation or L.I.D. Covenant <br />required for Federal Avenue. <br />5. The fire marshall shall be consulted to determine the adequacy of the <br />existing static water pressure in the area to meet fire flow demands. <br />6. Participation in the program to fund the north end sanitary sewer inter- <br />ceptor is required. 'fhat fee is $1,053. • <br />7. Stormwater runoff shall be conveyed to the sewer main on Nassau with <br />on-site detention, sediment removai %nd a gas trap provided. <br />8. Public Works Department approval of a temporary erosion control plan <br />is required before commencing construction activities; Plan to also include <br />quality, quantity and location of fill. <br />9. Exposed soils must be reseeded within 30 days of construction completion <br />and between construction phases. <br />Amend Checklist As Follows: <br />II - 13 - b, c, d, and f- change to yes. <br />Information For The De�•eloper• <br />A fire hydrant is required within 200 driving feet but not closer than 50 feet of <br />atl buildings. <br />jcl <br />
