With the addidon of the following, the plans are <br />approved. <br />1. The Health District operadng peraut application process must be completed prior to opening for <br />business. <br />2. An indirect waste is required for the food pteparation sink, mechanical dishwasher, ice machine, <br />running water dipper well, and equipment in which food is placed. <br />3. A reduced pressure backflow Prevention device is nequired at the end of the copper water pipe <br />prior to the carbonadon devic�. <br />4. All f�wd service equipment must be listed by the National Sanitation Foundation (NSF) for its <br />intended use. <br />5. The Federil Refrigeradon Bakery Case (Model #3150SC), the Lang Pan Bella Grill and the <br />Cornelius "SpiriP' Beverage Dispenser are not on the current NSF lisdng. These items must be <br />verified as being NSF approved or must be replaced with NSF approved equipment. <br />6. Extra wall protection is required on walls behind sinks and food preparation tables. A 16 inch <br />high backsplash of plastic laminate, fiberglass reinforced plasdc or equal is acceptable. Plastic <br />coated hazdboazd is not acceptable. <br />A preoperational inspection is required prior to openin� for business. At the time of inspecdon the <br />construction of the food service establishmen! must be complete and all equipment must be in place. <br />Incomplete constniccion may result in a$60.00 reinspection fee. Please contact the Food Program <br />office about one week in advance to schedule an appointment.
