CITY OF EVERETT <br />USE PERMIT #12-88 <br />November 15, 1988 <br />Applicant: Sanford Wright, Jr. M.D. <br />2320 Rucker Avenue <br />Everett, Wa 98201 <br />Project Location: 2326 Rucker Avenue <br />Previous Use: A single family residence with its first <br />floor of 1500 square feet and its second <br />floor of 1100 square feet converted into a <br />clinic (see Use Permit X8-86). The <br />basement was not included in the Use Permit <br />as usable space. <br />Proposed Use: Convert the 1400 square foot basement into <br />clinic space to allow access for the <br />handicapped. The second floor will be used <br />for storage space with approximately 100 <br />square feet of the second floor to be used <br />for administrative purposes. <br />Zoning: R-4, Multiple -Family High Density <br />Parking Provided: The site plan submitted shows fourteen <br />off-street parking spaces. <br />Parking Required: The 4,000 square foot building requires 20 <br />parking spaces for clinic use (one space <br />per each 200 square feet of gross floor <br />area). <br />A permit for the above described change of use is hereby <br />granted with the following conditions: <br />1. Pave the alley to the west of the clinic as required <br />under Use Permit #8-86. <br />2. The basement must meet all building code requirements <br />including but not limited to providing a minimum of 2 <br />exits from the basement. A building permit must be <br />obtained for the proposed conversion of the basement to <br />clinic space. <br />3. Submit a site plan to the Planning Department that <br />shows the site with 20 parking spaces as required by <br />code. The spaces must be provided on site or within <br />300 feet of the site. If the twenty parking spaces <br />cannot be provided, a parking variance must be obtained. <br />4. Any future change of use will require review by the <br />Planning Department to determine if the new use meets <br />the intent of this permit or will require a new <br />application. <br />DENNIS L.
