Wood Chip Facility <br /> Tax Account No. 092905-2-008-0001 <br /> Dear Sirs: <br /> • <br /> At the meeting of the Board of Health on September 11, 1990, the issuance of a holding <br /> tank permit to Dunlap Towing,Inc. was approved, subject to Dunlap Towing, Inc. entering <br /> into an Agreement with Snohomish Health District, such Agreement setting forth the terms <br /> and conditions of issuing the permit. <br /> An Agreement has been prepared and two copies are enclosed. I have signed these on <br /> behalf of the Snohomish Health District. Please review this document and, providing your <br /> concurrence, have the Agreement signed on behalf of Dunlap Towing, Inc. and return a <br /> copy to the Sanitation Office. <br /> When the Agreement has been executed with a'copy recorded onto the title of the property <br /> and permit fee paid($250.00), the holding tank permit can be obtained in accordance with <br /> Snohomish Health District permit procedures. It is required that the Sanitation Office be <br /> advised when construction of the holding tank is completed so an inspection can be <br /> conducted. <br /> Please call Randal S.
