DOL DAPS Master Contract o. 8147 <br /> DAPS Contract No. 9«<S <br /> Contractor Contract No. <br /> ATTACHMENT B <br /> BUDGET <br /> COST PER DELIVERABLE <br /> CONSIDERATION <br /> 1. When DOL is requested to provide hard copies of records to USER, USER agrees to pay DOL a <br /> fee covering DOL's direct cost for copying records, not to exceed fifteen cents ($.15) for each <br /> photocopy, seventy-five cents ($.75) for each copy of microfilm, microfiche or imaged document, <br /> plus delivery costs. <br /> 2. When copies of records are requested, USER agrees to pay DOL a fee of one dollar and fifty <br /> cents ($1.50) for each certification affixed to any print or photocopy, plus delivery costs. <br /> 3. USER agrees to pay DOL a fee covering DOL's direct cost for computer-generated lists. The fee <br /> for each request shall be agreed upon prior to DOL disclosing the information. <br /> 4. DOL maintains the right to increase or decrease the fees for rendering service under this <br /> Agreement. Any amendment to the fees shall be subject to a change in the Agreement as <br /> provided herein. <br /> BILLING DETAIL <br /> DOL shall bill USER no less than quarterly for: <br /> The monthly fixed cost of$3.00 (Three dollars) for each registered employee, for SecureAccess <br /> (regardless of activity). This fee will be reassessed periodically and the costs will go down as more users <br /> gain access. <br /> The cost for each request for supplying hard copies of records as described above. USER shall <br /> reimburse DOL with in thirty (30) days of receipt of properly completed invoices. <br /> FUNDING CONTINGENCY <br /> In the event funding from state, federal, or other sources is withdrawn, reduced, or limited in any way after <br /> the effective date of this Agreement and prior to normal completion, either party may; <br /> • Terminate this Agreement without advance notice subject to renegotiation under those new funding <br /> limitations and conditions or <br /> • After a review of project expenditures and deliverable status, extend the end date of the Agreement <br /> and postpone deliverables or portions of deliverables. <br /> s <br /> File Name: 8147-DAP-DOL Interagency DSA.doc Page 9 of4] Printed: 7/14/2006 <br />
