DESCRIPTION/JUSTIFICATION OF AMENDMENT, MODIFICATION, OR CHANGE ORDER: <br /> The Office for Domestic Preparedness (ODP) has provided funds through FFY 2004 HSGP for planning, exercise, <br /> training, and equipment for local jurisdictions to prevent and respond to terrorist/weapons of mass destruction attacks. <br /> This amendment is necessary to amend the contract expiration date to reflect the State EMD approved 1 month <br /> extension of the grant, and to close out the Region One grant funding by reallocate the budgets for Planning, <br /> Exercise,Training and Equipment within Region One, for grant allowable activities which occurred prior to May <br /> 31, 2006. <br /> 11. AMENDMENT TERMS AND CONDITIONS: <br /> 1. Change to the contract expiration date from April 30, 2006 to May 31, 2006 <br /> 2. Increase the overall contract amount from $30,975 to $43,865. <br /> 3. Replace the Budget Sheets, with Revised Budget Sheets —Amendment B. <br /> No other changes are required. All other terms and conditions of the original contract and any <br /> previous amendments thereto remain in full force and effect. <br /> Snohomish County and the Contractor acknowledge and accept the terms of this contract amendment and attachments <br /> hereto. The rights and obligations of both parties to this contract are governed by this Contract Amendment Face Sheet <br /> and other documents incorporated herein by reference or attached and identified in the original contract document. A <br /> copy of this contract amendment shall be attached to and incorporated into the original agreement between the <br /> Department and the Contractor.
