Community Health Center $175,000.00 <br /> South Everett medical &Dental Facility <br /> Sub-Total $1,016,949.28 <br /> Total(Social &Non-Social Services) $1,162,483.28 <br /> Budget Sources <br /> 2000 Entitlement: $930,000.00 <br /> Reallocations <br /> CHIP Legal Services $7,595.22 <br /> Minor Home Repair $21,028.42 <br /> CDBG Administration $105,870.64 <br /> ARC Automatic Door Opener 2,655.00 <br /> Housing Hope Village $1,800.00 <br /> Housing Opportunity Fund $87,500.00 <br /> St. Vincent de Paul $2,000.00 <br /> Catholic Community Services $25.76 <br /> Providence Sexual Assault Center 4,008.24 <br /> Total $1,162,483.28 <br /> II. If the Department of Housing and Urban Development reduces this allocation, then each item <br /> will be reduced an equal percentage, including the Community Housing Improvement Program <br /> and Community Development Block Grant Administration. If a reduction is large enough to <br /> cause a burden, as determined by the staff, then a new budget will be returned to the City <br /> Council for their review and approval. <br /> IV. The Mayor is hereby authorized to make formal application to the U.S. Department of Housing <br /> and Urban Development for funding assistance in accordance with this Resolution and the <br /> requirements of such application. <br /> V. Any funding assistance received in 2000 is to be used for the above projects. <br /> VI. This Resolution will become part of the formal application to the U.S. Department of Housing <br /> and Urban Development. <br /> VII. All payments and income from the Housing Rehabilitation Program will be used for housing <br /> rehabilitation purposes. This is estimated at $400,000.00 for 2000. <br /> VIII. The capital projects will be in the form of loans with details of terms and conditions to be <br /> returned to City Council in a contract. <br /> IX. The required application sections and information are to be sent to the U.S. Department of <br /> Housing and Urban Development to complete all application requirements. <br />
