14 <br /> RESOLUTION NO. <br /> A RESOLUTION CONCERNING BILLING AND COLLECTION FOR MEDICAL <br /> TRANSPORT SERVICE FEES <br /> WHEREAS, Ordinance No. 2792-04 provides for the imposition of fees for medical <br /> transport services by the Everett Fire Department; and <br /> WHEREAS, there is a need to establish criteria and procedures for the provision of <br /> financial assistance to individuals with no or inadequate means to pay Everett medical <br /> transport fees; and <br /> WHEREAS, financial assistance for ambulance transport will be consistent with the <br /> requirements of the Revised Code of Washington and the Washington Administrative <br /> Code; <br /> NOW, THEREFORE, BE IT RESOLVED: <br /> Section 1: <br /> 1. Ambulance transport fees are the fees established by Ord. 2792-04 for the <br /> provision of emergency medical assistance to individuals requiring basic and/or <br /> advanced life support services. <br /> 2. Debt forgiveness is the act of canceling a debt owed to the City of Everett for <br /> services rendered either partial or in full. <br /> 3. Financial assistance shall refer to the act of providing medical assistance to <br /> persons without ability to pay for such services, either partial or in full payment. <br /> Section 2: <br /> 1. Full debt forgiveness will be provided to a patient with a gross family income at or <br /> below 100% of current published Federal Poverty Income Guidelines. <br /> 2. Partial debt forgiveness will be awarded according to the schedule in Appendix 4. <br /> The resulting responsibility may be adjusted by joint approval of the Fire Chief <br /> and City Treasurer (or designees), based on extraordinary nondiscretionary <br /> expenses relative to the amount of the responsible party's medical care <br /> expenses or limitations on the responsible party's future income earning capacity <br /> due to illness. <br /> 13 `- <br /> 1 <br />
