6 -6-.3— -Proposer's-Proposal-for laximum-Escalati-on-Rate:--The-Proposer'-s Proposals for fees-shall-remain firm and fixed through the first year of the agreement. Proposer shall bid a maximum annual escalation percentage rate for years 2 through 6, should the City opt to extend the agreement for years 4, 5 and 6. 6.4 Administrative Fees and Taxes Not to be Included: The Proposer shall not include in its Proposal prices the City's Administrative Fee or state or local sales taxes. 7.0 INSTRUCTIONS FOR PROPOSAL FORMS C,D,E,F and G 7.1 Proposal Certification Proposer shall certify that its Proposal will be valid for 90 days after submission. Proposer may be asked to extend this certification. Proposer shall complete, sign, and submit Proposal Form C. 7.2 Non-Collusion Affidavit Proposer shall complete, sign, and submit Proposal Form D. 7.3 Affidavit Regarding Criminal Record Proposer shall certify that no owner in its firm has been convicted within the past ten years of a crime involving dishonesty or false statements. The owners of the Apparent Successful Proposer shall be subject to a criminal records check. Proposer shall complete, sign,and submit Proposal Form E. 7.4 Affidavit of Ownership Proposer shall complete, sign, and submit Proposal Form F completely and accurately stating the names and addresses of all persons,firms,corporations,partnerships or other associations having any direct or indirect financial interest in the Proposer's business and the nature and extent of each such interest.
