B. National Objectives <br /> The Subgrantee certifies that the activities carried out with funds provided under this Agreement will <br /> meet one or more of the following CDBG National Objectives - 1) benefit low/moderate income <br /> persons, 2) aid in the prevention or elimination of slums or blight or, 3) meet community development <br /> needs having a particular urgency - as defined in 24 CFR Part 570.208. Briefly describe how this <br /> National Objective will be met. <br /> Minor Home Repair meets objectives 1) benefit low/moderate income persons, and 2) aid in the <br /> prevention of elimination of slums or blight. Specifically, the program meets these objectives by <br /> improving permanent residential structure benefiting low and moderate income persons. <br /> C. Performance Monitoring <br /> The City will monitor the performance of the Subgrantee against goals and performance standards <br /> required herein. Substandard performance as determined by the City will constitute non-compliance <br /> with this Agreement. If action to correct such substandard performance is not taken by the <br /> Subgrantee within a reasonable period of time after being notified by the City, contract suspension or <br /> termination procedures will be initiated. <br /> D. Levels of Accomplishment - Goals and Performance Measures <br /> In addition to the normal administrative services required as part of this Agreement, the Subgrantee <br /> agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> Work Orders July 2016— December 2016 = 7 90 <br /> January 2017 —June 2017 = 8 <br /> Total People or Households to be served Total unduplicated Everett Residents or <br /> Households to be served <br /> 90 90 <br /> *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, <br /> persons or households assisted, or meals served, here. If this is a construction project, please <br /> explain here how the improvements will be used and how they benefit the community, what the <br /> target population is for the project, etc. <br /> The Subgrantee agrees to track outcome performance measures as required by the U.S. <br /> Department of Housing and Urban Development and the City. The Subgrantee will report the <br /> outcomes to the City on at least a quarterly basis. Information and forms will be provided to the <br /> Subgrantee by the City for reporting this information. <br /> E. Staffing <br />
