Tdest <br />Everett, Washington 98204 <br />Re: Plan Review - Chris' Off Broadway <br />Dear Mr. Wheeler: <br />Your plans have been reviecaed by the Snohomish Health <br />District with the following comments: <br />(1) Unles� self-contained, all condensate drair.lines <br />from refrigerated case units and walkin coolers <br />shall be iridirectly connected to the sewage <br />system. <br />(2) Yermanently fixed artificial light sources with <br />shielding to protect against broken glass falling <br />onto food shall be installed to provide at least <br />20 foot csndles of light in walkin refrigeration <br />unit• <br />(3) [dalls :�n the restroom facility must be light colored, <br />smooth, nonabsorbent, easily cleanable and in good <br />repair. Concrete blocks, if used in this location, <br />must be covered. <br />(4) Toilet rooms and mopsink sha11 be provided with <br />hot/cold water temnered by means of a combination <br />faucet or mixing valve. <br />(5) tdith the absence of nroper utensil washing and hand <br />washi.ng facilities, the Snohomish Health District <br />can only approve those foods which are commercially <br />prepackaged and obtained from an anproved wholesale <br />source and heated solely by microwave oven. <br />(6; All building and fire codes for the City of Everett <br />must Ue follocaed. <br />(7) The Board of Health establishe.. a plan review charge <br />for the review of nlans and specificati,ons. The fee <br />schedule is $25.00 per hour. Your total review time <br />is one hour or $25.00. This fee must be paid before <br />the final field inspection can be called for and an <br />operating permit be issued. <br />It should be pointed out to you that anproval o£ the finished <br />installation, equipment,and facilities is sub�ect to the <br />final field inspection. Therefore, only when the final <br />field iaspection has been called for and approved cvill the <br />food service permiC be issued. <br />
