The following information is needed prior to further plan review. <br />L A list of equipment noting manufacturer and model numbers for all food service equipment <br />including counter top equipment. <br />2. Finish schedule for floors, walls, ceilings, counters and cabinets. <br />3. Pluribing details or scliedule. <br />�l. Copy of proposed menu and or grocery items. <br />5. Submit a tloor plan, drawn to scule, showing location uf all equipment, plumbine fixtures <br />and the like. The scale of the drawin�should be l/4 inch equals 1 tbot. <br />6. Please refer to the enclosed copy of Food Establishment Conswction Plan Guidelines for <br />additional information and sample floor plan drawing. <br />7. The pian rcview fee you submitted was $175.00. The plan review fee is $295.00. The balance <br />of S120.00 must be submitted. <br />Please note that prior to opening of the �rocery, after the Health District plan review process is <br />completed and construction is finished, the Health District permit application process must be <br />completed and a pre-operational inspection must be conducted. <br />Please contact me if you huv� any questions.
