Ed Diamond <br />I agree t❑ Furnish all materials and perform all labor necessary <br />to construct second -story addition to oxisting clinic according to <br />plans and specifications and including the following. <br />Kitchen and conference room sinks - 22 x 25 stainless steel <br />with Delta 100 faucet. <br />Hain bath tub - 5' fiberglass with Delta 642 tub and shower <br />valve. <br />Office bath and main bath toilets - K3500 EB, white. <br />Office bath and main bath sinks - K2916, white with Delta <br />522 center set. <br />Kitchen counters and !lath vanities - Formica Brand's Solids, <br />patterns, or marbles with standard backsplash and self edge. <br />Bath floors - mannington's Vinyl ease or Vegal Biscayne 11 or <br />Conjoleum's Valuflor. <br />-arpet on remaining floors - Salem's Gracious over 3/6" rebond <br />pad. Deduct $180.00 if vinyl is used in place of carpet in <br />kitchen. <br />Kitchen refrigerator - TA115 General Electric <br />Kitchen range - JA5-03 General Electric 21" <br />Conference room under counter refrigerator - Sears 46 G 91342N. <br />Conference room hot plate - Sears 34 G 6829. <br />Soffit to be cedar to match existing building. <br />Exterior of new addition to be stained same color as existing. <br />All interior walls and ceilings to be plasterboard, taped, <br />textured and finished with one coat of sealer and one coat <br />enamel. <br />Owner to obtain all necessary permits. <br />Light fixture allowance - $600.00. <br />All of the above work to be completed in a substantial and workman- <br />like mann-r according to standard practices for the sum of Fifty - <br />Eight Thousand Nine Hundred Oollars($58,900.00) plus Washington <br />State Sales Tax. Payments to be made according to standard bank <br />draws as the work progresses. The entire amount of the contract <br />to be paid within ten days after completion. Owner to carry fire <br />and other necessary insurance.
