Beverly Park Substation — Concrete Demolition <br />WO# 379123 <br />Present: <br />Will Blanchard (PUD) <br />Craig Lia� (PUD) <br />Craig DeYoung (PUD) <br />Bob Eden (PUD) <br />Pre-Construction Meeting Minutes <br />1/24/2012 <br />Erika Wittman (PUD) <br />Cliff Lindman (City of Everett) <br />A!ike Thompson (PUD) <br />Zack Rose (ICI) <br />Caleb Armstong (ICI) <br />Chris Jensen (ICI) <br />Overview <br />District, Interwest Construction and City personnel attended a pre-construction meeting to discuss <br />the upcoming work at Beverly Park Substation. <br />Points of discussion <br />1) A safety plan must be submitted to Mike Thompson for approval before work can begin (ICI). <br />• ICI to prepare plan and submit to Mike Thompson, cc Will Blanchard. <br />i. Completed 1/26!12 <br />2) Caleb (IC1) would like to drive over the concrete covered trench in lieu of the drive path <br />propased per plan. <br />• From visual inspection by ICI and PUD the drive path may be adjusted to accommodate <br />access. The drive path may be adjusted under the authority of the safety watch <br />assigned to this project coordinating with Craig DeYoung. <br />i. 1/25/11—Zack (ICI) has submitted a sketch bridging the trench. The district has <br />respo� ' witii questions and comments. <br />3) Zach (ICI) would like to substitute rumble strips for the quarry spall entrance. <br />• Approved provided the dimensions of the rubble strips once assembled are ae least the <br />size of the construction entrence as defined per plan and positioned such that vehicle <br />traffic must travel over them. Creig DeYoung to confirm. <br />4) eob (PUD) talked about the results of the testing on site and provided a report detailing the <br />findings. The remnt testing and inspection dia not find contamination above Model Toxics <br />Control Act Method (MTCA) clean up levels in tested areas for the soil or concre[e. Historical <br />records frrnn Puget Sound Power and Light (PSPL) indicate that concrete pads that supported oil <br />filled equipim nt were previously decontaminated and removed. <br />. During constructien there is always the possibility of encountering something <br />unexpected. Should ICI encc�nter anything concerning, stop work in that area and <br />notify Craig DeYoung who will serve as the liaison to coordinate with District personnel <br />to �Jetermine appropriate action. <br />i. This contrad does not indude soil removal, soils are to remain on site. <br />
