During our site vWk we <br />documented our observations pbotographioelly suil took several material samples. Uis <br />repod describes our observations attd provides our opinions regWing the basement wall <br />failure. <br />IN7RODUMON <br />The subject residence In a two-story wood•ftamed structure approximately 80 years old <br />with a daylight bnemem atldi single story addition at fat Wor level at the east end. 7Tre <br />internal plan dimensions of the main portion of the house are apptoximately 28A x 246. <br />You reported to us that the north side of the baeema►d wall collapsed on October 15. <br />2000. <br />OBSERVA71ONS <br />During our site visit we made the following observations: <br />I . 71w Boil beneath the footpath all along the north side of the residence has given <br />way for a width of spprmdruately 31. <br />2. The besartwat well at the north aide, which is approximately 71t tall, has collapsed <br />into the basement area. <br />3. The collapse Includes I►Il ue of the concrete well, one larger aeration at the center <br />of which has holm Into Ism piwa. <br />4.
