All work shall be carried out in accord-ince <br />with the attached permit. <br />Conditions of approval are that sidewalk which was broken as a result of <br />construction activity, be removed and replaced; the existing driveway curbcut <br />be restored and replaced; that now extruded curb and a cleanout inlet be in- <br />stalled. Six (6) inch concrete driveway section installed at sidewalk. <br />Enclosed is a copy of the City rules and regulations governing work within <br />the public rights -of -way. Please call the Engineering Department Construction <br />Division (259-8811) prior to starting this work so that an inspection can <br />be made. <br />This permit is valid for a period of sixty (60) days from date of issue.
