4 <br /> RESOLUTION NO. <br /> A Resolution extending health benefits to the domestic partners of <br /> employees and their dependents. <br /> Whereas, the extension of domestic partnership health benefits is consistent with the <br /> City of Everett's commitment to recruiting and retaining a diverse workforce; and <br /> Whereas, there is a desire by the City of Everett to extend health benefits to eligible <br /> domestic partners and their eligible dependents; <br /> NOW, THEREFORE, it is hereby resolved by the Council of the City of <br /> Everett: <br /> To the extent an insurance carrier agrees to extend such coverage, medical, vision and <br /> dental benefits shall be extended to the Domestic Partners of City of Everett employees <br /> and their eligible dependents effective January 1, 2008. <br /> a. "Domestic Partner"means a partner in a relationship between persons of the <br /> same or different sex who: <br /> (1) are each other's sole domestic partner and live with each other in the <br /> same residence in an exclusive and emotionally committed relationship <br /> similar to a marriage with the intent to continue doing so indefinitely; and <br /> (2) are not married to or legally separated from anyone else; and <br /> (3) are at least eighteen(18) years old and mentally competent to consent <br /> to contract; and <br /> (4) are not related by blood to a degree of closeness that would prohibit <br /> legal marriage in Washington state; <br /> (5) are jointly responsible for each other's common welfare and have <br /> shared financial obligations, including at least three of the following: <br /> (a)joint mortgage or lease; <br /> (b) designation of domestic partner as beneficiary for life <br /> insurance; <br /> (c) designation of domestic partner as beneficiary for retirement <br /> contract; <br /> (d) designation of domestic partner as beneficiary in employee's <br /> will; <br /> (e) designation of domestic partner as agent under durable property <br /> and health care powers of attorney; <br /> (f)joint checking account; <br /> (g)joint credit account; <br /> (h) Washington State registration as a domestic partnership; and <br /> (6) are not in engaged in the relationship solely for the purpose of <br /> obtaining benefits coverage. <br /> 14 <br />
