r <br />U <br />ATTACHMENT A <br />SCOPE OF SERVICES <br />1. Work to be Performed <br />The Sultan River Diversion Dam has been in place since 1930. It was originally used to divert water <br />from the Sultan River into Lake Chaplain for the City of Everett's water supply. Since construction <br />of the Jackson Hydroelectric Project, the dam is used primarily to provide fish flows downstream of <br />the project and emergency water supply to the City. The dam is a concrete gravity overflow, structure <br />25 feet high and 120 feet wide. The diversion dam has an ogee crest at elevation 655 feet. The <br />impoundment created by this dam is limited to a few acres. It classified as a low hazard dam and as <br />such is not required to have a Part 12 Inspection, as judged by FERC, though FERC inspections the <br />dam nearly annually. <br />In order to assess the current condition of the diversion dam and associated equipment for continued <br />safe and reliable operation, MWH will perform the following tasks: <br />■ Obtain from the City and review information such as project drawings, previous reports, <br />equipment data, operations information, applicable CAD files, etc. Upon notification to start <br />work, MWH will provide a complete list of required information. <br />■ Make a one -day site visit to the dam, accompanied by the City, in order to assess the condition of <br />the concrete dam structure, gates and gantry structure, and appurtenant features. In order to do a <br />thorough inspection of the dam, the will City provide a small boat and operator to enable <br />inspection and measurement, if possible, of eroded areas at the toe of the dam. <br />■ Interview City and PUD operations staff, during the site inspection, regarding operational and <br />maintenance issues related to the dam. <br />■ Write a brief assessment report describing the condition of the existing dam and operational <br />features of the dam and estimate life of existing features for continued functionality into the <br />future. Make a prioritized list of recommendations for potential repairs and upgrades at the dam. <br />The report will be submitted to the. City in draft form for comments and then finalized. <br />Independent computations will not be performed as to the stability of the dam and load -carrying <br />capability of project structures and equipment. <br />2.
