1 <br /> Exhibit "A" <br /> City of Everett <br /> East Marine View Drive <br /> PHASE IV: Construction Observation and Administration Services <br /> Supplemental Agreement No. 1 <br /> INTRODUCTION <br /> This project involves the development of corridor improvements along East Marine View Drive, <br /> from the northbound I-5 off ramp to the SR 529-Broadway Ave on/off ramp within the City of <br /> Everett. Improvements include roadway enhancements and non-motorized trail facilities to connect <br /> similar facilities to the south and north of the project area. This supplemental agreement will reflect <br /> changes to the following areas of the original scope of services. <br /> SCOPE OF SERVICES DEFINED <br /> Phase IV—Construction Observation & Administration Services <br /> The scope of services for construction engineering services by the Consultant for the federally <br /> funded East Marine View Drive project is supplemented with the following. <br /> General Assumptions <br /> • Perteet replaced City provided Construction Observer with a Senior Construction Observer <br /> Lead effective May 12, 2008 until the completion of the project. <br /> 2. CONSTRUCTION ADMINISTRATION <br /> 2.17 Maintain files. <br /> • Project photographs (digital format). Perteet furnished digital cameras to <br /> Construction Observers. <br /> Summary of Office Administration (Record Keeping): <br /> 1. Provide one full time on-site Construction Manager, one Senior Construction Observer, <br /> and one Construction Observer. <br /> 3.
