Obtain permit to widen entryways. <br />2. Remove existing red curb, sidewalk, curb and gutter and concrete entries. <br />3. Prep and plain two new entries in concrete to meet city specs, and to allow for a more gradual <br />approach to entryways so cars don't `bottom out'. New entryways will be 35 feet long by 11 <br />feet wide. <br />4. Excavate grass, sod and dirt to approx. 10 inches in depth for asphalt extensions. <br />5. Place and compact 6 inches of 5/8 minus crushed rock sub base in two lifts of 3 inches. <br />6. Apply weed killer per specs. <br />7. Place and compact 3 inches of Class B asphalt. <br />8. Seal joints with AR-4000 tar sealant. <br />9. All cuts required will be sawcut. <br />10. Work to be completed in two phases. <br />We hereby propose to furnish labor and materials <br />complete in accordance with the above <br />specifications for the sum of $as shown above <br />with payment to be made in full upon completion <br />of the work unless other terns are arranged prior to <br />the beginning of work. NOTE: Ifthe above <br />proposal includes asphalt repair, that price includes <br />removal of asphalt up to 2 inches in depth. Should <br />additional asphalt or sub base have to be removed <br />and replaced, an additional cost will be incurred. <br />This proposal may be withdrawn if not accepted <br />within 20 days. <br />All material is guaranteed to be as specified. All work is <br />to be completed in a workmanlike manner according to <br />standard practices. Any alteration or deviation from the <br />above specifications involving extra costs will be <br />executed only upon written orders and will become an <br />extra charge over and above the estimate.
