MITIGATED <br />DETERMINATION OF NON -SIGNIFICANCE <br />SEPA #54-95 <br />McDonald's Restaurant <br />DESCRIPTION OF PROPOSAL: The proposed action is issuance of building and public <br />works permits to allow construction of a 3,590 square foot fast food restaurant with 48 parking <br />stalls and drive -up window. The listed requirements are placed in response to our <br />review of this information: <br />AGENCIES WITH JURISDICTION <br />A Hydraulics Project Approval (HPA) from the State Department of Fisheries or Wildlife <br />may be required for this project due to discharge of stormwater to Wood Creek. A copy <br />of the HPA or a letter from the Department of Fisheries or Wildlife stating that an HPA is <br />not needed should be included along with the plans for Public Works review. Note: <br />More stringent detention and water treatment requirements than the City would impose <br />may be placed on the project as a condition of the HPA. <br />2. A permit is required from the Snohomish County Health District. Please contact Rick <br />Zuhalka, at 339-5250 for more information. <br />MITIGATION OF ADVERSE IMPACTS BY SEPA <br />Any grading/fill on this site shall be done so as to not impact the surrounding properties. <br />(SEPA Earth, Land and Shoreline Use Policies.) <br />
