(I <br />AGENCIES WITH JURISDICTION <br />Federal Aviation Administration -FAA review is required for construction projects <br />exceeding certain thresholds. The proposed 150, foot steel monopole may exceed the <br />100 to 1 criteria as contained in the Federal Air Regulation, Part 77.13(2)(1). The <br />Applicant must fill out FAA Form 7460-1 and submit it to the Northwest Mountain <br />Region of the FAA. <br />MITIGATION OF ADVERSE IMPACTS BY S'e"PA <br />Any grading/fill on this site shall be done so as to not impact the surrounding properties. <br />(SEPA Earth, Land and Shoreline Use Policies.) <br />The lead agency for this proposal has determined that it does not have a probable <br />significant adverse impact on the environment. n Environmental <br />on assumes ct Statement <br />tat en with <br />is <br />not squired under RCW 43.21C.030(2)(c). <br />State law and City ordinances related to general environmental protection_ including but <br />not limited to right-of-way improvement requirements, drainage, etc. This information <br />on file with the lead agency. This <br />Mitigated Determination of Non -Significance is specifically conditioned on compliance <br />with the conditions attached hereto which are incorporated by reference as if fully set <br />forth herein. <br />This MDNS is issued under 197-11-340(2); the lead agency will not act on this proposal <br />for 14 calendar days from the date below.
