SE, Everett <br />Dear Mr. Olson: <br />Your plans have been reviewed for compliance with the Rules and Regulations of the <br />State Board of Health and with the policies of the Snohomish Health District. With the <br />addition of the following item, the plans are approved. <br />Extra wall protection is needed in the kitchen behind sinks, worktables and <br />dishwashers. Plastic laminate, fiberglass reinforced plastic or equal material <br />should be used and should cover walls from the top of worktables and sinks up 16 <br />inches. In the area of the dishmachine, extra protection should run from the floor <br />to the top of the dishwasher. <br />If there are any significant changes or additions to your layout or equipment, the <br />Snohomish Health District must be notified. <br />A pre -operational inspection is required prior to opening for business. Please contact <br />the Food Program office about one week in advance to schedule an appointment. <br />This will insure compliance with the Rules and Regulations of the State Board of <br />Health for Food Service Sanitation (WAC 246-215). <br />Please contact ma if ycu have any questions.
