TIMING LIMITATIONS: The project may begin immediately and shall be completed by <br />December 31, 2006. PROVIDED: <br />a. Work below the ordiriary high water line shall only occur between July 1 and October 15 of 2005 <br />and 2006. <br />2. NOTIFICATION REQUIREMENT: The Area Habitat Biologist listed below shall be contacted by <br />phone or e-mail (holsegh@dfw.wa.gov) within seven days of completion of work to arrange for a <br />compliance inspection. The notification shall include the permittee's name and the control number <br />for lhis HPA. <br />3. Work shall be accomplished per plans and specifications approved by the Washington <br />Department of Fish and Wildlife entitled Waterford Lane, 18th Avenue West and dated June 22, <br />2005, and addendum dated August 18, 2005, except as modified by this Hydraulic Project <br />Approval. A copy of these plans shail be available on site during construction. <br />4. Prior to starting work, temporary filter fabric, siraw bale, or pea gravel-filled burlap bag check <br />dam(s) shall be installed downstream. Accumulated sediments shall be removed during the project <br />and prior to removing the check dam(s) after completion of work. <br />5. The culvert shall be installed in the dry or isolation from the stream flow by the installation of a <br />bypass flume or culvert, or by pumping the stream flow around the work area. Care shall be taken <br />so that the stream below the project area is never dewatered. At least half the (low of the stre�m <br />shall be maintained ir the downstream reach at all times. <br />6. A sandbag revetment or similar device shall be installed at the bypass inlet to divert the entire <br />flow through the bypass. <br />7. A sandbag revetment cr similar device shall be installed at the downstream end of the bypass to <br />prevent backwater from entering the work area. <br />Page 1 of 6 <br />
