CONTRACT NO.81232 C' <br /> '.I <br /> those rates are attached as"Attachment A"actual costs billed will be based on the current rate of the <br /> individual doing the work and that may vary from those averages cited in"Attachment A". If work is <br /> actually performed when the individual is not scheduled to work,the actual costs will be computed at a time <br /> and a half basis per the current labor contract In addition to direct labor costs,billings will include actual <br /> labor driven benefits mandated by law(e.g.FICA,etc.)as well as those mandated by the prevailing labor <br /> agreements(e.g.employer paid retirement contributions)which are based on actual hours of work paid. <br /> 6. Insurance:Minimum insurance requirements are: (A)comprehensive general liability,broad form,$500,000 <br /> combined single limit each occurrence for bodily injury and property damage;(B)comprehensive <br /> automobile liability(owned and non-owned),$500,000 per person,$500,000 per occurrence for bodily <br /> injury,and$500,000 per occurrence for property damage; (C)statutory cover Workers'Compensation. <br /> These insurance requirements may be met by maintaining a fully funded self-insurance program. <br /> 7. Government-Furnished Property: The following Government property shall be delivered to the contractor: <br /> NONE <br /> 8. Contract Contents: In addition to this Schedule,the contract consists of: <br /> a. Statement of Work entitled PRND Pilot Project <br /> b. General Provisions,dated 1/15/09 <br /> c. Attachment A,City Of Everett Police Department Rates Effective 1/1/09 <br /> 11. Integration: The Agreement contains the entire understanding between the parties,and there are no <br /> understandings or representations not set forth or incorporated by reference herein. No subsequent <br /> modifications of this agreement shall be of any force or effect unless in writing signed by the party claimed to <br /> be bound thereby. No communications,written or oral,by other than a Battelle Contract Representative shall <br /> be effective to modify or otherwise affect the provisions of the agreement. <br /> III.
