Project: WFP Clearwell No. 2 Change Order No.5 Effective June 23,2009 Exhibit A- Description of Changed Work Change Order#5 Create New"Bid Iran#8'L entitled,"Change Ord" 45", iifthe lump sum amount of-$64,315.77, which shall incorporate Work Change Directives 020,060, 061, 062 and 066 per the following: 1. Work Change Directive 020—Delete Finished Water Flow Meter Vault floor section and weep ring(Detail 4/1C-503) and reinforcing detail EM 070 GC-704. Decease contract by$1,290.00. 2. Work Change Directive 060—Install a storm drain pipe northwest of Clearwell No. 1. Increase Contract by$1,354.00. 3. Work Change Directive 061 —Delete roadway guardrails from the new road to be installed between Clearwell No. 1 and Clearwell No.2. Decrease Contract amount by $34,400.00. 4. Work Change Directive 062—Reimburse City of Everett for PUD charges that should have been paid by the Contractor. Decrease Contract amount by $ 48,964.00. 5. Work Change Directive 066—Correct settlement out-of-roundness and add a filler band so the existing 84-inch Filtered Water pipe and the new 84-inch Filtered Water pipe that have different outside diameters could be connected with butt straps. Increase the Contract by$18,984.23. JUSTIFICATION: 1. Work Change Directive 020—It was preferred by the City of Everett that the Finished Water Meter Vault be free-draining instead of having the concrete floor. The reinforcing was shown in the electrical duct only in the manhole detail and is not used anywhere else. 2. Work Change Directive 060-The added storm drain pipe was installed to prevent ponding of water in the new site grading. 3. Work Change Directive 061 —Guardrails included in the design are not used anywhere else on site and not desired by the City of Everett plant staff. 4. Work Change Directive 062 —The City of Everett was invoiced by PUD and inadvertently paid for work that should have been paid by the Contractor. The Work Change Directive 062 reimbursed the City. 5. Work Change Directive 066—The existing 84-inch Finished Water line between the Filters and the Clearwell Control Structure through the years had gone out-of-round by 5-inches.The existing 84-inch pipe and the new 84-inch pipe have different exterior diameters. This required that the existing pipe be slowly jacked into roundness and that a flat band be welded to the outside of the smaller diameter pipe to allow welding the butt strap connection between the two pipes.
