PLEASE NOTE, THE GLOBAL SERVICES AGREEMENT MUST BE SIGNED BY AN AUTHORIZED <br /> OFFICIAL OF YOUR COMPANY AND RETURNED TO UL BEFORE UL MAY PERFORM SERVICES <br /> UNDER THIS QUOTATION. <br /> The Program Terms and Conditions are intended to cover charges associated with our inspection of your <br /> aerial lift devices and/or pumpers and/or ground ladders,to determine that equipment conforms to the <br /> appropriate provisions of the National Fire Protection Association (NFPA)Standard No. 1911, "Standard <br /> for the Inspection, Maintenance,Testing, and Retirement of In-Service Automotive Fire Apparatus", 2007 <br /> Edition; and/or NFPA 1932, "Use, Maintenance, and Service Testing of Fire Department Ground <br /> Ladders,"2004 Edition. <br /> Devices covered under our Inspection Program are not eligible for Listing by UL. Shortly after completion <br /> of our work, a copy of our inspection and test Report will be provided. Upon completion of the inspection, <br /> if the equipment is found to comply with the applicable requirements, UL will issue a Certificate of <br /> Inspection. <br /> Charges are due and payable without discount upon presentation of our invoice. You will receive a copy <br /> of the Inspection and Test Report prior to receiving our invoice. <br /> The current charge for testing aerial devices is $ 1,075.00 per unit. The charge for testing ground ladders <br /> is $ 2.50 per linear foot. These are the current charges for testing services. If pricing changes, you will <br /> receive a letter with the updated pricing. <br /> Please direct your attention to the paragraphs that are applicable to your equipment. <br /> AERIAL DEVICES: <br /> Inspections of aerial devices use non-destructive testing methods that can include magnetic particle <br /> inspection, ultrasonic inspection, dye penetrant inspection and hardness inspection as appropriate for the <br /> materials involved. Also, the equipment is required to be subjected to operation and load tests as <br /> specified in the above referenced Standards. <br /> NFPA 1901 requires that the manufacturer provide to the purchaser technical information needed to <br /> conduct aerial service tests under NFPA 1911. A copy of this document should be available to the UL <br /> Representative in order to assure that the proper test criteria are applied. If your apparatus predates this <br /> requirement and you did not receive this document, UL can provide you with a form for this purpose. <br /> UL will make every effort to complete all testing in one day. However, circumstances may make <br /> completion of testing in one day impractical, if not impossible. These circumstances may include <br /> weather, or structural problems with the aerial device. <br /> Inclement Weather: <br /> If weather conditions, such as high winds or freezing cold interrupt testing, twn_aptionsare_availabla_The <br /> Authority Having Jurisdiction (AHJ) may accept an incomplete set of tests. In this case, the report would <br /> list all completed tests and inspections, along with their results. The report would also list all tests that <br /> were not completed due to the inclement conditions. A Certificate of Inspection to NFPA 1911, stating <br /> that the test work was incomplete, can also be issued. <br /> The AHJ would also have the option of requesting a completion of the inspection. In this case, UL would <br /> schedule a second date that is mutually convenient to complete the testing. The fire department would <br /> receive a second report, showing that all inspections were completed. A Certificate of Inspection to <br /> NFPA 1911, this time showing that all work was completed, would be issued. <br /> Structural Damage: <br /> If structural damage is identified which makes completion of testing unwise, some tests will not be <br /> conducted on the day of test. Typically, the load test is not completed. Other tests may include Water <br /> 79 <br />
