SUBMITTAL CHECKLIST FOR <br /> ADULT FAMILY HOME INSPECTION <br /> PERMIT SUBMITTAL REQUIREMENTS: <br /> 1. Two (2) copies of a floor plan, drawn to scale and including all oi the components <br /> shown on the attached sample <br /> 2. Completed Permit Application <br /> 3. Inspection fee of$54.50 <br /> 4. All documents must be clearly legible for microfilming. <br /> "Applications are accepted betwoen the hours of 8:00 AM and 4:00 PM only <br /> To apply tor an Adult Family Home inspeclion, you will need to fiil out a City of Everett Permit <br /> Application form, provide two (2) copies ot an adequate tloor plan and pay a fee of$54.50. An <br /> adequate floor plan will include ali lhe requirements shown on the altached sample. You will also <br /> need to show how you can provide three (3) off-street parking spaces. Staff can assist you by <br /> copying a site plar� from our mapping program. <br /> If you are doing remodel +vork to your home to alter or allow for additional bedrooms or living <br /> space, you will need to apply for a residentiat building permit.
