Pny changes to the pl�ns submitted June 11, 2013 will void this approval. <br />If the establishment continues to be in operation during the remodel, dust be riers must be installed to ensure lhat <br />food proteclion and saFety is not compromised. The establishnent will be ,equired to discontinue operations if any <br />disruption in water, electrical, or sewer service occurs, due to the remooel. Proper handwash stations must be <br />kept ac�essible and functional during all hours of food service establishment operetion. <br />A preoperational inspection is required prior to opereting permit Issuance and approval to open for <br />business. At the time o( inspection the construction o( lhe food service eslablishrnent must be complete and all <br />equipment must be in place and in proper operating condition Incomplete construction or equipment opera6on <br />will result in a 5168.00 re-inspection fee. Contact the Food Program office a minimum of one week In <br />advance to schedule an appointment for the preoperetional inspection.
