May 13, 1999 <br />Y//L/L N/O/PXS <br />M & M Assc :iates <br />1910 120th Pl SE <br />Everett, WA 98208 <br />RE: EXPIRING PERMIT & CERTIFICATE OF OCCUPANCY <br />Job Address: <br />Permit: <br />1902 120th Pl SE, Tenant Guardian Capital <br />C59849 (Building & Plumbing for tenant improvement) <br />According to our records there has been no request for inspeaion in the lut 180 days, and no <br />Certificate of Occupancy hu heen issued for this projett. Although the Building portian of <br />the permit received final approval on September 10, 1998, there hu been no Plumbing <br />inspettion. The Uniform Building Code provides that all permiu expire by limitation if the <br />work authorized by the permit is suspended or abandoned at any time aher work is <br />commenced for a period of 180 days. If you wish to prevent the permit from expiring <br />without documencation of completion and without the Certificace of Occupancy, you may: <br />1) Request fnal inspeaion of the plumbing work, <br />or 3) Submit a written request for a 180 day extension. <br />Please respond within 10 days. After that time the file will be sent to our Records <br />Depanroent for micro6lming without final approval.
