a <br /> Permit Conditions <br /> General. <br /> 1. The permit for alley closure will only be valid from 1:00 p.m.to 5:00 p.m.on May 15,2010. <br /> Street barricades will be available and placed from the alley entrance to the alley behind 2308 <br /> Lombard. <br /> 2. It may not be represented or implied that the event is City sponsored or sanctioned. <br /> 3. At its sole option,City may verify concurrences by adjoining property owners;lack of <br /> concurrence by any adjoining property owner may be cause for revocation or non-issuance of <br /> permit. The applicant must provide proof that all residents in the 2300 block of Lombard(west <br /> side)and all residents in the 2300 block of Oakes(east side)are in concurrence with the alley <br /> closure by May 11,2010. <br /> 4. Permit may be revoked at any time after issuance if,in the sole opinion of the City staff, <br /> . satisfactory compliance with permit conditions is not achieved or maintained. <br /> 5. Material changes to closure plan presented to the city as of May 4,2010 may only be made by <br /> with the City's approval. In its sole discretion,the City may withhold approval. <br /> 6. There shall be no service of any alcoholic beverage in the closure area. <br /> Health/Safety <br /> 7. Applicants must dean up;the City will not provide deaning equipment or labor. All dean up will <br /> be completed prior to street opening,but no later than 6:00 p.m. Applicant will be charged for <br /> any fees related to dean-up if not completed to the satisfaction of the City. The City Street <br /> Department will conduct a clean-up inspection. <br /> 8. The permit and closures may be suspended or terminated for life/safety reasons or for failure or <br /> refusal to timely comply with reasonable direction by city personnel,such as police or fire,or <br /> failure to comply with permit conditions. <br /> Vendors • <br /> • 9. No vendors. <br /> • Financial <br /> 10. No financial conditions. • <br /> Traffic <br /> 11. The street closure area is approved as designated on the attached street dosure plan contingent <br /> on approval from adjoining neighbors. <br /> 12. Applicant shall arrange and pay for placement of all necessary barricades and signs at the alley <br /> • entrance and midway to the alley at the start of the dosure on the south side. Applicant shall <br />• provide any and all labor for manning traffic barricades at intersections. Barricades will always <br /> • • be manned by applicant and moved as needed for emergency access or other needs. Applicant <br /> will contact Traffic Engineering at 425-257-8800 for arrangement of barricades. <br /> 13. Applicant will remove all traffic barricades and signage no later than 6:00 p.m.and place all City <br /> barricades and signage at the intersection of the alley and 2314(off street)for pickup by the City. <br /> • <br /> Risk Management <br /> • 14. Applicant must jointly and severally defend,indemnify and hold the City harmless from any and <br /> • all claims of any kind in a form provided or approved by the City Attorney's Office. <br /> • 15.
