5. Must not be found to be addicted to the use of intoxicating liquor, dangerous drugs or narcotics within two years of the date of application; 6. Must have submitted to a medical examination at the applicant's expense by a licensed physician and filed with the city clerk a certificate from said physician certifying his fitness as a for -hire driver. Such examination shall be required at least every four years but not for renewals of such license; provided, however, the city clerk may at any time at her/his discretion require any licensee to be so examined and to secure such a certificate or renewal thereof, any applicant who fails to pass a satisfactory medical examination may be re-examined after thirty days from the date of original examination, and if following such re-examination the certificate herein required is issued, the applicant may be licensed in accordance with the provisions of this chapter provided all other requirements are met; 7. Must not have been convicted of any felony crime pertaining to alcohol, controlled substances, fraud, gambling, physical violence, prostitution, racketeering, robbery, theft or that is otherwise directly related to the applicant's ability to properly operate as a for -hire driver, and the time elapsed since the conviction is less than ten years; 8. Must not have been convicted of or forfeited bail to any crime pertaining to alcohol, controlled substances, fraud, gambling, physical violence, prostitution, racketeering, robbery, theft, or that is otherwise directly related to the applicant's ability to properly operate as a for -hire driver, and the time elapsed since the conviction is less than three years; 9. Must not have been convicted of any criminal traffic offense within a period of two years preceding his application for a for -hire driver's license; 10. Must not have been found to have committed any two traffic infractions within twelve months preceding an application for a for -hire driver's license, whether it be a new application or an application for renewal thereof; 11. Must show proof that he is covered by an insurance policy which meets the minimum requirements as set forth in this chapter; 12. Must successfully complete a written examination administered at the time of initial application and at such other times as required by the city clerk. The examination will test the applicant's knowledge of the ordinance requirements dealing with fare determination, driver -passenger relations, conduct including the applicant's ability to understand oral and written directions in the English language, vehicle safety requirements and driver regulations, and a satisfactory geographic knowledge of the city; and 13. Must not have been convicted of a crime for which he is required to register as a sex offender pursuant to RCW 9A.44.130.
