Attach list of any repairs or corrections completed in connection <br /> with above certification. A "Standard Nntice of Work Completed" or a report form <br /> indicating no infestation may be submitted as certification. Note: All repairs must <br /> be completed in a professional manner and in compliance with local building codes. <br /> Vc-14 Q Q (a) This commitment is subject to receipt of clear certificate(s) for (electrical, <br /> plumbing, roofing, and heating). The required certificate s) is (are) being ordered <br /> and will be paid for by HLID.
