SPECIAL TERMS AND CONDITIONS <br /> INTERAGENCY <br /> FEDERAL FUNDS <br /> 1. ACKNOWLEDGEMENT OF EPA FUNDING <br /> The Contractor agrees that any reports, documents, signage, videos, or other media, developed as <br /> part of projects funded in whole or in part with federal funds under this Agreement, shall contain the <br /> following statements: <br /> "This project has been funded wholly or in part by the United States Environmental <br /> Protection Agency under assistance agreement PO-00J093-01-0. The contents ofthis <br /> document are those of the author and do not necessarily reflect the views and policies of the <br /> U.S Environmental Protection Agency, nor does mention of trade names or commercial <br /> products constitute endorsement or recommendation for use. Grant funds are awarded by <br /> Puget Sound Regional Council and Washington State Department of Commerce." <br /> 2. AGREEMENT MANAGEMENT <br /> The Representative for each of the parties shall be responsible for and shall be the contact person for <br /> all communications and billings regarding the performance of this Agreement. <br /> The Representative for COMMERCE and their contact information are identified on the Face <br /> Sheet of this Agreement. <br /> The Representative for the Contractor and their contact information are identified on the Face <br /> Sheet of this Agreement. <br /> 3. COMPENSATION <br /> COMMERCE shall pay an amount not to exceed thirty thousand dollars ($30,000) for the <br /> performance of all things necessary for or incidental to the performance of work as set forth in the <br /> Scope of Work. Contractor's compensation for services rendered shall be based on the following <br /> rates or in accordance with the following terms: <br /> EXPENSES <br /> Contractor shall receive reimbursement for travel and other expenses as identified below or as <br /> authorized in advance by COMMERCE as reimbursable. <br /> Such expenses may include airfare (economy or coach class only), other transportation expenses, <br /> and lodging and subsistence necessary during periods of required travel. Contractor shall receive <br /> compensation for travel expenses at current state travel reimbursement rates <br /> 4. QUARTERLY REPORTS <br /> Contractor shall submit a status report, on a form provided by COMMERCE, every three (3) months <br /> describing the progress made on the scope of work outlined in "Attachment A" and performance <br /> measures to be determined with EPA during the term of the contract. Reports are due fifteen (15) <br /> calendar days prior to the due date for reports to EPA, to be determined. Quarterly Reports shall be <br /> submitted to COMMERCE and may be provided electronically. <br /> 5. FINAL REPORT <br /> In addition to the quarterly status reports, Contractor shall submit a final report, which is due thirty <br /> (30) calendar days after the expiration or termination of the contract. The report shall be submitted to <br /> COMMERCE and may be provided electronically. The report shall generally contain the same <br /> information as the status reports, but should cover the entire contract period. <br /> 2 8 <br />
