7. The Director may consult with the Department of Fish and Wildlife before accepting <br />the habitat assessment as final, and if recommended by the Department of Fish and <br />Wildlife may require preparation of a Habitat Management Plan. <br />All habitat assessments submitted under the requirements of this chapter shall, at a <br />minimum, include the following: <br />a. The parcel number(s) of the subject property. <br />b. A map showing the location of the site and the site address of the subject property, if <br />one has been assigned by the City. <br />c. The date and time when the site evaluation for the habitat assessment was conducted <br />and the date when the habitat assessment was prepared. <br />d. The credentials of the professional fish or wildlife biologist who prepared the habitat <br />assessment. <br />e. The mailing address and phone number of the property owner and the fish or wildlife <br />biologist that prepared the habitat assessment. <br />f. A detailed description of the project. <br />g. A detailed description of the vegetation on and adjacent to the site. <br />h. Identification and a detailed description of any critical fish or wildlife species or <br />habitats, as set forth in this chapter, on or adjacent to the site and the distance of such <br />habitats or species in relation to the site. Describe efforts to determine the status of <br />any critical species in the project area, including information on survey methods, <br />timing, and results of surveys for species or suitable habitat identification. <br />i. Include any information received from biologists with special expertise on the <br />species or habitat type, such as WDFW, Tribal, USFS, or other local, regional, <br />federal, and university fish, wildlife and habitat biologists and plant ecologists. <br />Include any such conversations in the habitat assessment and cite as personal <br />communication. <br />j. An assessment of the project's direct and indirect potential impacts and cumulative <br />impacts on the subject habitat, including water quality impacts. <br />k. A discussion of potential mitigation measures that would avoid or minimize <br />temporary and permanent impacts, proposed mitigation measures, contingency <br />measures, and monitoring plans. <br />1. The city may require that the applicant request a separate evaluation of the site by <br />WDFW staff to confirm the findings of the habitat assessment. <br />9. The Department shall review the habitat assessment and either: <br />a. Accept the habitat assessment as complete and include any recommended mitigation <br />measures necessary to reduce impacts to the critical fish and wildlife habitat <br />conservation areas or effected species as project requirements; or, <br />b. Require preparation of a Habitat Management Plan if the habitat assessment <br />indicates potential unmitigated adverse impacts to the critical fish and wildlife <br />habitat conservation areas or effected species. <br />D. Habitat Management Plan. Habitat Management Plan means an activity proposed by a <br />public agency or private entity, and approved by the Planning Director, within an area <br />51 <br />
